| Additional Funds Form |
| Co-applicant applying for additional funds loans must complete separate request form. The mortgage account should never have been in arrears for the last 12 months preceding the application for Additional Funds. |
| Increased Mortgage Payment Form |
| This form applies to mortgagors who consistently pay more than the required mortgage payment, and who wish for the excess to be paid to the principal balance on their accounts. |
| Pay-off statement request form

| Before you pay off your NHT mortgage, you will need a pay-off statement. To obtain this statement, please complete the attached form and submit it by way of the 4 options listed on the form. This form becomes effective January 1, 2010. The first pay-off statement requested is free of cost. There is a charge of $500 for any further requests. |
